Medical photography is primarily used to record detailed and accurate images to assist clinicians in diagnosis and monitoring of patient conditions, disease and progress. With your specific consent they could also be used for teaching and research purposes.
The department is open for patient photographs Monday to Friday
09.30 to 16.00. Please telephone the department on 01702
385093 to make an appointment. This will normally be offered
within 48 hours.
Please remember to bring the clinical photography consent form
with you to your appointment.
You will be asked to consent to having your photographs taken at
the hospital. You have the right to know how your photographs are
used, and you have the right to change your mind about the
permission you have agreed to at any time. If you wish to withdraw
your consent please contact the department as soon as possible
after your attendance. Please note that it will not be possible to
remove photographs from the public domain once they have been
Your doctor or clinical nurse specialist would have
discussed the need to refer you to Medical Photography
and will have given you a yellow refferal form which you
should read, sign and bring with you to the department at your
appointment time. For patients under the age of 16 a parent or
guardian's consent is required.
Your health records
The main purpose of your photographs will be for your own
personal health records. It may be necessary to forward your
photographs to other hospitals in relation to your on-going

Your consultant or doctor may speak to you about the possibility
of using your photographs for teaching and/or publication. They
will explain the purpose and intended media and will ask for your
consent. Most medical publications will appear online
and you should be aware that images may appear there too. This also
relates to any non-medical photographs that may be taken of you for
We offer a service to other NHS Trusts, private patients and
local and national solicitors for recording of medical condition or
scars/injury resulting from an accident or assault. These
photographs are taken in accordance with national guidelines and
are admissible in court. There are charges for this service. Please
