Discharge team

Following your treatment in hospital, you will be discharged when the consultant or health professional in charge of your care decides that you are well enough to leave.

You will be involved in planning your dischargeand will be able to discuss your needs with the discharge team. We will work to ensure that you have everything that you need for a full recovery when you return home.

The departure lounge

The departure lounge is based in the day assessment unit and provides a waiting area for patients leaving hospital. It operates between 09.00 and 19.00 for patients able to sit in a chair. Clothes can be provided, as can any necessary snacks to take home until care arrives.

Out of hours departures

Safe discharges from hospital should take place between 07.00 and 21.00. Transport is available via the day assessment unit weekdays until 21.00, and via the discharge coordinator at until 17.00 at weekends. Discharge from hospital should be avoided where possible after this time, particularly for elderly patients, unless it is in the patient's best interest. It is the responsibility of the named nurse to ensure that safe discharge plans are in place if a discharge is planned outside these hours.

Colaborative Care Team (CCT)

For the Southend area

This is a team of carers and rehabilitation staff who provide rehabilitation for patients in their own home.

  • The patient must be assessed as able to cope overnight without funded care
  • All patients must have a named social worker prior to transfer
  • Patients are assessed by the CCT prior to acceptance

For the Castle Point / Rayleigh / Rochford areas

This is a rehabilitation-only team, funded by the PCT to provide rehabilitation for patients in their own home. They do not provide social or personal care, but do supplement a social care package where there is a rehabilitation need. Referral is from the rehabilitation team via the discharge team fax number.

  • The patient must be assessed as able to cope overnight without funded care
  • All patients must have a named social worker prior to referral
  • Patients are assessed by the CCT prior to acceptance
